Attention An Achievable Dream College Students!
The following two documents must be completed EACH time scholarship money is requested. Instructions for downloading forms and returning forms are listed below. Please follow all directions to ensure quick processing.
POLICIES:
Scholarship funds will not be dispersed until FINAL GRADES from the previous semester are submitted. Students must maintain a 2.0 GPA to be eligible for scholarship funds.
Scholarship checks will ONLY be processed on the 15th and 30th of each month. All scholarship requests must be submitted five days in advance.
If you would like to request scholarship funds for the purchase of a computer, please complete the attached forms and include the specifications for the computer. AAD, Inc. will purchase the computer with your scholarship funds. Computer orders will be placed on August 15 and January 15 of each year. Computer requests must be submitted ten days in advance.
For students attending a community college, documentation must be provided that they registered three weeks prior to the payment deadline. This policy is to ensure that financial aid is processed by the payment deadline.

To download form, click on the link corresponding to your desired form. Choose 'save to disk' when the popup window shows to download the form to your hardrive. Fill out the form completely and fax back to 757-873-4887 or email as an attachment to lvreeland@achievabledream.com
